Certified Nursing Assistant - Per Diem
Job DescriptionNurse Assistants are directly supervised by a Nurse Teamleader but scheduled/evaluated by the Shift Supervisor. Nurse Assistants provide personal care to residents following resident’s Care Plan Approaches and they monitor and document resident’s activity, appearance, documenting care provided and levels of activity. Must work well under stress or tight deadlines. Must work well with supervisors, co-workers, patients/residents, family members and visitors. Provides all care tasks abstracted from the residents Care Plan. Performs or assists residents with personal hygiene using established procedures indicated. These may include bathing or showering, peri-care, dressing, hair grooming, oral care, cleaning and trimming fingernails/toenails (except diabetics). Distributes meal trays from carts, cuts foods, assist/feed residents using established procedures, documents resident’s fluid/ food intake and picks up meal trays. Ensures prescribed snacks and supplements are offered. Changes bedfast and chair-fast resident’s body position to maintain proper position/body alignment. Applies restraints, supports, and pads to prevent falls, contractures and pressure sores. Follows prescribed programs of preventive skin care, recognizing symptoms of skin breakdown. Takes resident’s vital signs and records the data as prescribed. Assists residents with voiding and elimination using commodes and when not possible, urinals, bedpans, or disposable briefs. Answers resident call lights, generally within 5 minutes. Collects specimens such as urine, stool, and sputum for laboratory analysis using established procedures based on the Universal Precautions (UP) and correct labeling technique. Monitors each resident for food and fluid intake and bowel and bladder output. Records amounts as part of shift ADL charting. Encourages and motivates residents to be out of bed, dressed, active, and to reach their highest practical level of self care and emotional well being. Assists in lifting, moving, transferring, and ambulating residents. Follows safe work and care practices to protect themselves, residents, visitors, and other staff. This includes using prescribed body mechanics, obtaining assistance to lift or transfer residents, using gait belts, lifting devices, and applying prescribed restraints. Use infection control procedures based on UP. Assists in maintaining the resident’s room, closet, and other work areas neat and orderly. Monitor for changes in resident’s status, including vital signs, level of physical activity, emotional well-being, cognitive ability, comfort or pain. Report these changes to the Nurse Team Leader. Attends shift meetings and in-service programs. To be able to propritize your skills to meet the residents needs, in a calm professional manner. Charts on each of their resident’s Activity of Daily Living accurately on the ADL Record Form prior to the end of the shift. Other Duties Assigned by Supervisor . Preferred SkillsEducation: No requirement but High School graduate or GED preferred. Experience: None Certifications: Have a current California Nursing Assistant Certificate, or complete the Certification course. Skills/Ability: Be knowledgeable of common resident care techniques as demonstrated during the initial department orientation. Be able to communicate effectively with residents to the degree the resident’s ability allows. Be able to follow written and verbal instructions and communicate resident’s condition to other staff. Have sufficient strength and endurance to fully participate in resident lifting and transporting. |
