At the Comprehensive Care Center, we make transitions as easy as possible for our skilled nursing and post-acute rehabilitation residents. Whether you've been recently discharged from the hospital or are in a medically fragile condition and need specialized care, we're committed to helping you with your activities of daily living and rehabilitative needs.

Our admissions process includes a comprehensive evaluation of each resident’s treatment needs with a keen eye to individualized discharge plans. Our rehabilitative treatments are tailored specifically to meet each resident’s goal of increased independence. We are dedicated to ensuring a safe and speedy transition home for everyone eligible.

Our CCC stays include the following:

• 24-hour nursing care and treatment
• Quarterly care plan meetings, discharge planning, and social services
• Frequent updates for family members
• In-house physical, speech, and occupational therapy
• Measures to prevent falls and improve balance
• Access to a registered dietitian and specialty diets
• In-facility activities throughout the day
• Intravenous therapy
• Medication management
• Pharmacy consultation
• Recreational therapy
• Access to hairstylist and barbers
• Pet visits
• Transportation to medical appointments

Our Rehabilitative Goal

At the CCC, we are committed to getting you back to feeling like yourself again. We excel at keeping your mind and body active with comprehensive recreation, social, spiritual, and cultural offerings.

Advanced nursing and extensive rehabilitation services can help you grow stronger and more independent after leaving the hospital. You will be able to practice and perfect your independent living skills before leaving the CCC, giving you the confidence to return home safely.
