New Policy Regarding Distribution of Medical Records

Due to the onset of the COVID-19 virus and in order to adhere to restrictions put in place by the California Department of Public Health to prevent the spread of the disease, Lompoc Valley Medical Center (LVMC) and Lompoc Health are requesting patients to refrain from seeking the release of their medical records in person.
Medical records will be released according to state and federal laws and in a manner that can be accomplished in a timely yet safe manner for everyone during this infectious disease outbreak.
Patients are encouraged to use the LVMC or Lompoc Health portals to obtain their records whenever possible. Patients without existing access may obtain a portal form at lompocvmc.com. The form can be submitted along with legal photographic identification to receive access.
Authorization forms to release protected health information and instructions for completion are also available at lompocvmc.com. Once completed, they may be mailed to:

Alternatively, faxed to (805) 737-3386, emailed to medicalrecords@lompocvmc.com, or delivered to the front entrance of LVMC or any Lompoc Health location.
During this period of pandemic restrictions, Health Information Management (HIM) staff will not accept requests in person.

All requests for records will be completed within 15 days or as designated by law.
During this pandemic, medical records will only be distributed by either U.S. Mail or secured email.
